Gathering Data: Most Common and Least Common Illnesses

So I'd like to try and figure out how common certain illnesses are and categorize them into 4 Category's: Common, Uncommon, Rare, and Extremely Rare. I have set up a Google Form to help me gather information.

You may fill it out as many times as you like so long as the information is as accurate as possible.

As far as super common illnesses like open wounds go you can guess within reason if you don't remember how many times you have gotten it in your pack (I'm 99.9% sure that Open wound is the MOST common ailment in the game).

Please don't count the illness you get while starting the herbalist quest It will mess up the numbers.
